About penis enhancement surgery

By penis enlargement surgery, also called phalloplasty, the aim is to achieve two objectives, making the penis thicker (girth enhancement) and penile elongation. The surgery typically is done at an out-patient basis and takes around 2 hours. The term phaloplasy, however, has a wider meaning. It is also used about reconstruction or correction of the penis, regardless the reason for doing it.


Enhancement of the penile shaft thickness or circumference using transplanted fat tissue

Girth enhancement is accomplished in the following way. First the surgeon aspirates fat from fat-rich body parts, for example the pubic mound, abdomen and "love handles". He then injects this fat underneath the skin of the penile shaft. In this process the surgeon aims at creating as smooth, symmetrical and cylindrical a shaft as his skill of hand-work permits.

By this process it is essential that the injected fat cells fasten at the right place, and that they do not die. Mechanical impact may make the fat slide away from the place where it was injected before it has fastened. Cell death causes the fat to be reabsorbed again and the enlarging effect disappear.

The objectives of enhancing the thickness may be reached at various degrees, sometimes the result is excelent, at other times rather poor. With a poor result, the girth enlargement is not totally permanent, and the process may produce lumps, bumps and clumps.

Surgical enhancement of the circumference, neither do produce any real growth of the original penile skin and penile tissue, the penil skin is only pumped up by the fat injected. Therefore the injected fat produces a greater internal pressure of the penis, and will make the penis stiffer and more entrenched. This may feel allright, and not have any great negative consequences on the penile function, but may also cause major problems. Furthermore, this fat encapsulation does not have any enervation. Therefore it may cause an insulation of the sensorial mechanisms of the penis from the stimulation by sex, thus making the penis numb and make a loss of feeling.

Some other possible drawback is that the transplanted fat cells feel soft, somewhat like female breast tissue.

Some surgeons claim that they can increase the penis circumference by 50 % in the best cases, but by such results, the sensory isolation problem and the stiffness or pressure problem will tend to be greater. In order to avoid problems, most surgery customers must be content by much less.


Enhancement of the penile thickness or circumference using tranplanted skin tissue (allograft demal matrix transplantation)

This is a fairly new techique. By this techique skin connective tissue from doner skin delivered through tissue banks is transplanted under the skin of the penile shaft. The girth of the penile shaft can be widened up to 40% this way. The tissue has all living cells removed before it is used this way, and after the transplantation new cells from the resceiver's body itsef grow into the transplanted tissue and makes it living again.

This kind of penile thickness enhancement probably gives better results than fat transplantation because it results in a firmer penile structure. It seems to give good lasting cosmetic results and results regarding penile girth enhancement. Still many nerve cells will be cut with this techique even though one tries to spare nerves. Also this kind of techique will give a shielding effect between the penile skin and the inner parts of the penis so that stimulatory movements may not come through to the inner parts as easily as before or in other ways as before.


Elongation of the penis

Penis lengthening procedures are very different from the procedures used to increase the girth of the penis. The spongy tissue in the penis that fills with blood, the corpora cavernosa and the corpus spongiosum surrounding the urethra, normally extends from the visible penis backwords under the scrotum and all the way to the prostate area near the anus. In some cases, half of the erectile bodies may be hidden under the scrotum and in the male perineum. Penis lengthening is effected by releasing a part of this concealed tissue which effects an apparent lengthening of the penis.

The lengthening of the penis is achieved in the following way: The surgeon makes a small angle-shaped incision on the pubic mound at the penile base (within the cover provided by the pubic hair). Then the suspensory ligaments that ancher the hidden part of the penis to the pubic bone is divided. This releases a portion of the penis that normally lies hidden under the upper part of the scrotum and this portion, about an inch and a half, rises from the scrotum into visibility, and moves somewhat foreward. By this procedure, the newly visible part of the penis also have to be covered by skin from the abdomen.

This surgery may be combined with the use of a penis strecher some hours a day for some months in order to gain permanent results.

Such a surgery do not in reality lengthen the penis, but causes some hidden part of the penis to come forward. Cutting ligaments will of course also have some effect on the stability and angle of the erected penis, the erected penis may wobble when being in action. After the surgery, the penis will also naturally seem to have change place somewhat downward. These things may or may not be felt as a problem. The resulting net effect may be good or bad.


Quality of outcome from penile augmentation surgery

Penis augmentation surgery does not as explained above really elongate the penis or add real penile tissue to make it thicker. It will however make the visible penis longer and thicker, and this may give a better functionality. The surgery may however cause scarring of the penile skin, making the penis less elastic and giving an irregular cosmetic result.

Some men are contented by the outcome of the surgery, while the frustration of others about the surgical result have made them sue their doctors. Because of the uncertainties with this surgery, the American Urological Association (AUA), the American Society for Aesthetic Plastic Surgery (ASAPS) and the American Society of Plastic Surgeons (ASPS) have policy statements against cosmetic penils enhancement surgeries.



Some penile condition where surgery sometimes takes place

Chordee

is a condition in which the penis curves downward (that is, in a ventral direction). The curvature is usually most obvious during erection, but resistance to straightening is often apparent in the flaccid state as well. In many cases but not all, chordee is associated with hypospadias. Chordee is often treated surgically allready in the infancy.


Phimosis

The foreskin in young boys cannot often be pulled back because the foreskin is adhering to the glans. The opening in the tip of the foreskin may also be very narrow in young boys, and this may also make it difficult to pull the foreskin back to expose the penile head. Usually the adherances dissolve and the opening widens so that pulling back will be possible before the boy is around 3-5 years old.

If the opening is too narrow to be pulled back in older boys or in adults, the condition is called phimosis. Phimosis can make it difficult by intercource and make old smegma and bacteria assamble and give rize to infection.

Sometimes the foreskin is wide enough to be pulled back, but difficult or impossible to pull foreward again. By this variant the foreskin pulled back can be stucked at that position, a condition called paraphimosis. Paraphimosis can hinder the blood circulation to the penis head so that the penis head is destroyed or badly injured.

The narrow ring in the foreskin can be elastic so that it widens upon stretch or hard and inelastic due to excessive fibrous connective tissue.

A hard and inelastic phimosis is often the consequence of repeated infections or injuries to the tip of the foreskin.

Phimosis can often be treated by stretching each day for a certain period, especially if the narrow area is elastic. It is orten treated with topical agents containing steroid hormons, eventually combined with stretching. The hormones loosen the inelastic connective tissue.

If these types of treatment do not cure the condition, surgery is sometimes performed. One kind of surgery is a dorsal split, where the overside of the forskin is cut at the tip, so that the opening gets wider, but the cut will make the foreskin look somewhat unnatural.

Another kind of surgery is foreskin plasticity where one performs more complicated cuts and sew together to make a forskin of natural shape.

Still another option is partial circumcission, where the tip of the foreskin is cut Off. If the foreskin is also too long, the procedure can leave a foreskin with an average natural shape.

The most drastic treatment for phimosis is full circumcission. This procedure was common earlier, but it is seldome done nowadays if there are not simultanous other reasons to want this procedure.


Frenulum breve

Frenulum breve is the condition in which the frenulum, which is an elastic band of tissue under the glans penis that connects to the prepuce (foreskin) and helps contract the prepuce over the glans, is short and restricts the movement of the prepuce. The frenulum is concidered normal if it is sufficiently long and supple to allow for the full retraction of the prepuce during erection so that it lies smoothly back on the shaft of the erect penis. The frenulum is comparable to the small band between the tongue's lower surface and the lower jaw.

Frenulum breve may or may not be felt as a problem. It can make bad feelings during sex and make it difficult to clean the glqns, or it can be felt as an advantage to have a short frenulum that hinders completely withdrawel of the foreskin. A penis where the foreskin is not completely withdrawn is often better to masturbate than one where the foreskin gets pulled completely back.

If it constitutes a problem, it can corrected surgically by a procedure called frenuloplasty or by circumcision, including removal of the frenulum. Such surgery should not be done if the short frenulum is not felt as any problem.

Circumcission that also completely removes the frenulum takes away much tissues with the ability to recogize sexual stimuli and can be felt as having more disadvantages than advantages. The frenulum pulls at the glans during sexual acts and thereby increases the stimulus. Frenuloplasty or circumcission will remove this physiological mechanism.


Hypospadias

By hyposapdias the urethra opens at the underside of the penis instead at the tip. It is usually corrected in infancy by reconstructiong the urethra from the original opening to the tip. This procedure do not allways give a permanently stable result, and must often be reviced later when the penis is fully grown.


Epispadias (hyperspadias)

In this condition the urethra opens at the upper part of the penis. It is often a component of a bigger combination of malformations where also the bladder can be open (bladder extropphy).
